« Protéger la Prochaine Génération » : promouvoir l'abandon des mutilations génitales féminines dans le district sanitaire de Kayes au Mali

Selon l’OMS, « Les mutilations génitales féminines (MGF) recouvrent toutes les interventions incluant l’ablation partielle ou totale des organes génitaux externes de la femme ou toute autre lésion des organes génitaux féminins qui sont pratiquées pour des raisons non médicales » [1]. En plus d’être un problème de santé publique, les MGF sont une violation des droits humains des filles et des femmes. Équilibres & Populations (France) et l’AMSOPT (Mali) développent un projet pilote qui vise la promotion de l’abandon des MGF dans le district sanitaire de Kayes au Mali. Il s’agit de démontrer que le changement social est possible et reproductible. La stratégie se base sur la diffusion du changement dans des communautés reliées par des liens maritaux. Le dispositif de communication s’appuie sur la promotion de la santé et des droits humains. Le projet a été construit sur la collaboration avec les acteurs politiques et sociaux afin qu’ils s’approprient les changements en cours. En France, le projet travaille avec les migrants issus des villages d’intervention. L’évaluation a montré que 80 des 250 villages du district ont déclaré l’abandon des MGF. Un débat positif émerge dans les communautés mais les messages doivent être renouvelés. La réponse du système de santé a été renforcée. Mille trois cent vingt-huit (1 328) femmes ont bénéficié d’une prise en charge des complications de l’excision. Réfléchir à l’évolution du projet afin d’atteindre ses objectifs devient nécessaire. Si la recherche de financements a pu être un obstacle, l’obtention de fonds pluriannuels va permettre une plus grande souplesse dans la gestion du projet. Plus large que l’abandon des MGF, la vocation du projet est de renforcer le statut et la santé des femmes et de leur donner les moyens de défendre leurs droits.

English abstract

Female genital mutilation (FGM) comprises all procedures that involve partial or total removal of the external female genitalia, or other injury to the female genital organs for non-medical reasons. FGM is not just a public health problem, but a violation of the human rights of girls and women. Equilibres & Populations and AMSOPT are developing a pilot project designed to promote abandonment of FGM in the Kayes health district by demonstrating that social change is possible and reproducible. The strategy is based on the organized diffusion of change among communities linked by marital ties. The communication process is based on promotion of health and human rights. The project collaborates with political and social actors so that they can take charge of the ongoing changes. The project also works in France with migrants from villages involved in these interventions because they also have an important role to play. Evaluation of the results showed that 80 of the 250 villages of the district declared abandonment of FGM. A positive debate is emerging in the communities, but messages must be repeated. The response of the health system has been reinforced, as 1328 women received medical care for complications of excision. The project now needs to be further developed in order to achieve its goals. Although the search for funding has been an obstacle, the acquisition of pluriannual funds will provide greater flexibility in management of the project. Beyond abandonment of FSM, the project also wants to reinforce the status and health of women and give them the possibilities to defend their rights.
